As a Physical Therapist at VNA Health you will provide excellent rehabilitative care to patients under the supervision of a supportive clinical management team.
As a PT, you will help develop and implement the plan of care, assess patient needs, provide treatments, educate patients, family members and caregivers re:
use of appliances and equipment, and supervise Physical Therapy Assistants and Home Health Aides, as needed.
As a Home Health PT, you will meet with patients where they reside, and work effectively with the other members of your interdisciplinary care team to achieve positive patient care outcomes.
You will have the opportunity to provide in-service trainings to staff as well as meet with other PTs for peer discussion / learning.

This may include, but will not be limited to:
Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
Evaluates home environment.
Assesses for muscle strength, mobility, gait, ROM--potential for rehab.
Directing physical therapy treatment.
Instructing patients and families/caregivers in the use and care of therapeutic appliances.
Determining priority needs for physical therapy Reporting to physician patient's reaction to treatment or changes in condition.
Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
May train patient in the use of prosthetic device.
Supervising home health aides and Physical Therapy Assistant when such personnel participating in patient's rehabilitation regime.
Identifies patient and family/caregiver needs for other home health services and refers as necessary.
Prepares and submits clinical and progress summaries based on the attainment of goals.
Meets the average daily productivity standards as set by the agency.
Participates in discharge planning for patient.
Prepares and submits a clinical progress summary based on the attainment of goals directed by Organization policy.
Provides physical therapy consultation to home families/caregivers when indicated.
Provides in-service education programs for nursing organization personnel as needed.
Participates in peer consultation process.
Assists with chart audits if schedule allows.

All clinical staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs and departmental holiday rotation.
POSITION QUALIFICATIONS Current and active California Physical Therapist license.
Two years of appropriate experience as a physical therapist.
Community/home health experience is preferred.
Current CPR certification.
Licensed driver with insured automobile - in accordance with state and/or organization requirements.
Demonstrates good verbal and written communication and organization skills.
PHYSICAL REQUIREMENTS Ability to lift 50 lbs.
Ability to push/pull 50 lbs.
Ability to stoop, bend, and squat.
Ability to walk up one flight of stairs.
Ability to walk one mile.
Ability to sit for one hour, minimum.
Visual and audio acuity.
